The Washington County Health Department, Meritus Health and Healthy Washington County are asking all Washington County residents (and those who work, play, or pray) to take a short survey about community health issues in Washington County, Md. This online survey is part of the community health needs assessment (CHNA) process. The information shared will be used to identify health priorities and to address these priorities in Washington County through community action over the next several years.
All who live, work, play or pray in Washington County are encouraged to take this anonymous, 10-minute survey.
